The purpose of this research is to investigate the pathology of the stakeholder survey system in Yazd Gas Company. A qualitative research approach with an exploratory design was adopted. Purposive sampling was used to select 11 experts, including managers and employees of the gas company, for semi-structured interviews. Data analysis was conducted using thematic analysis, which resulted in the extraction of 245 initial codes, 28 sub-themes, and 7 main themes. The seven main themes identified were: negative stakeholder perception of surveys, insufficient attention to surveys by the company, poor stakeholder cooperation in responding, wak company infrastructure, weak survey infrastructure, external factors affecting the company, unusable analysis. The findings of this study can be of importance for improving the survey process in the organization.
